HOW THE Registration Process WORKS

The registration process for STEM YEA student participants is completed through a local Rotary Club that has allocated funds for the STEM YEA tuition and appoints a STEM YEA Local Club Rep to coordinate STEM YEA registration. The STEM YEA Local Club Rep works with a middle school principal or other school administrator to identify interested students. The students, nominated by the school, are current 7th grade students (2024-2025 school year).

Student Registration Forms are included below. The Registration Packet forms should be completed, signed, scanned and forwarded to registration@stemyea.com

Student Registration Packet

This packet contains the forms and waivers for STEM YEA Academy Registration. Click this link to fill out the form. Complete the form. Press the SUBMIT button to record your entry. (No data is saved unless you press SUBMIT). You will receive an email confirming the information you entered. The email has an EDIT RESPONSE button which allows you to re-access the form and modify it. Remember to press SUBMIT to save your changes. Save the emails you receive as they verify information entered and allow you to modify the record if needed. After completing the Registration form you should download the Waiver, Permissions, and Agreement document. Print and review the document. Sign, scan and E-mail it to registration@stemyea.com.

Physical Examination Form

A completed 6th-grade Health History Form is acceptable. If that is not available, download and print this PDF and have it completed by your doctor. Either the 6th-grade form or the Health History Form must be scanned and sent to registration@stemyea.com in order for you to attend STEM YEA Academy. Please put Medical Form in the subject line of your E-mail.

Immunization Requirements

All students must have completed all pediatric immunizations as recommended by the CDC. Medical reasons for not getting a particular vaccine will be considered on an individual basis. There will be no exceptions for vaccine avoidance for philosophical or religious reasons.
